Player Safety


EGHA is dedicated to player safety. 

Player Medical Form

All teams should have players complete a player medical form at the start of the season. Forms to be kept on hand by the head coach or safety person. These forms can be important should there be an injury or need to transport a player to hospital if a parent or guardian is not available.

Hockey Canada Return to Play

A gradual return to play is recommended for all players after an injury. Please refer to the Hockey Alberta Return to Play Guide for more information.

The Hockey Canada Return to Play form is to be completed by a doctor clearing a player to return to play after injury. This form should be returned to your player's coach, with a copy being sent to safety@egha.ab.ca.

Hockey Canada Injury Report

This report should be completed within 90 days of an injury, with a copy submitted to Hockey Alberta. Please also submit a copy to safety@egha.ab.ca

Concussion Awareness

It is important that we know the signs of a concussion. An excellent tool, EGHA recommends all parents review the Concussion Awareness page on Hockey Canada's website.
